1- A certified copy of the national ID card of the applicant's father or mother

2- A copy of the consular card of the applicant's father or mother

3- Certified copy of the birth certificate of the applicant's father or mother

4- Birth certificate of the applicant

5- Fee of 200 DKK

6 - Return fees of 100 DKK by registered mail if the applicant resides out of Copenhagen
